בכצ'ט בישראל
לא קיימת הערכה רשמית לגבי
מספר החולים בישראל, אולם סביר להניח
שכרגע מאובחנים 500-1000 חולים במדינה.
בכצ'ט נפוץ יותר בקרב גברים, והם
נוטים לפתח מחלה קשה יותר. המחלה אינה
נדירה בילדים. המחלה פוגעת בעקר
בישראלים ערבים ויהודים לא-אשכנזים,
אולם היא קיימת גם בקרב יהודים
אשכנזים.
קיימת אסוציאציה חזקה בין
חולים ישראלים ופלשתינאים לבין HLA B-5,
בעיקר HLA B-51, אולם גם HLA B-52 .
בישראלים ערבים קיימת גם אסוציאציה
חזקה נוספת, ל MIC A-6.
אחוז ההתרחשות המשפחתית גבוה
בקרב כל המגזרים הישראלים, בעיקר אלו
הנושאים את אנטיגן HLA B-5, ומתבטאת
בעובדה שלרבים מהחולים בני משפחה
הסובלים מפצעים חוזרים ונשנים בפה,
וכמעט כל בני משפחה אלה נושאי HLA B-51 או
B-52 בעצמם. בקבוצת החולים הנושאת B-51 (נושאי B-52 לא נבדקו)
נמצאו כימוטאקסים נויטרופילים במידה
מוגברת, ומחסור של אימונוגלובולינים
בצומת הדרמו-אפידרמי .
אין הבדלים גדולים בין
התסמינים של חולים מקבוצות אתניות
שונות בישראל, למעט במקרים הבאים:
1 ערבים סובלים מבעיות עיניים
חמורות (דלקת ענבייה אחורית) לעתים
קרובות יותר מאשר יהודים.
2 בקבוצה היהודית, המחלה חמורה
ביותר ביוצאי צפון-אפריקה, הסובלים
מיותר בעיות עיניים (בעיקר דלקת
ענבייה קדמית) ומיותר דלקות פרקים,
דלקות בכלי הדם, פקקת ורידים עמוקה
ובכצ'ט של מערכת העצבים הכללית.
ניתוח לגורמים העלה אפשרות של
אסוציאציה בין בעיות עור מסויימות ,
או פקקת ורידים לתסמינים אחרים של
המחלה, ולמין החולה, גיל בזמן האבחון
ו-HLA. בנוסף לכך, FV (פאקטור וי. ליידן)
יכול להוות סיכון נוסף להתפתחות
מחלות עיניים, ובמיוחד חסימת ורידים
ברשתית, ועלול לתרום לראיה הלקויה
בחולים אלה.
מציאותה של תגובת פתרגיה אינה
מקושרת עם סיכון גבוה יותר של
תסמינים כלשהם, ואין בה בכדי לעזור
בחיזוי מהלך מחלה חמור יותר, אולם
תגובת פתרגיה מופיעה כמעט בכל חולה
ישראלי.
